APPOINTMENT SCHEDULING: Medical Arts Family Medicine is committed to providing quality care to our patients. To ensure timely continued care, we encourage patients to schedule routine follow up appointments two-three months in advance. PREVENTATIVE/ANNUAL PHYSICALS: Many insurances dictate that if any problems are discussed or prescriptions are generated from this wellness exam, your copay, or deductible will then become due. Wellness exams are to focus on health promotion activities, such as updating screenings and recommended vaccines. Please discuss your wellness benefits with your insurance provider and notify our office if such benefits are not available to you. Due to limited AM availability, we recommend you schedule your annual physicals 3-5 months in advance.
NO SHOW POLICY:
A “no show” is someone who misses an appointment without notifying our office.
If it is necessary to cancel your scheduled appointment we ask that you notify us as soon as possible. Appointments are in high demand, and your early cancellation will give another person the ability to have access to timely medical care.
Excessive no shows/frequent last minute cancellations may result in a discharge from our practice.
FINANCIAL/INSURANCE: It is important for you to provide our staff with correct and complete insurance information. Please check to see that your provider is identified as your primary care physician and familiarize yourself with your plan's copays, immunization policies, and referral procedures.

REFERRALS: Please allow 72 business hours for processing of insurance referrals. We are unable to process any referrals for issues that have not been addressed by your primary care provider.
REFILLS: Please allow 48-72 business hours for processing of refills. We kindly ask that you do not wait until you are out of medications to request refills. Refill requests may also be left on our secure voicemail (please be sure to include the name of medication and pharmacy).
If you are on medications for treatment of ADD/ADHD, Insomnia, Depression, or Anxiety you are required to be seen once every 6 months.
If you are on narcotic medications for pain management, you are required to be seen once every 3 months.
In general, all patients are required to be current patients seen within the last 12 months to get a medication refill. However, some patients may require labs or an office visit more frequently.
PRIOR AUTHORIZATIONS: Due to the high volume of prior authorizations we receive, we ask that you allow 48-72 business hours for our staff to initiate requests through your insurance. Depending on your insurance, this process may involve several steps and it may take an additional 72 business hours for our office to receive a final determination.
Neither the pharmacy, nor the provider can guarantee that your insurance company will approve the medication.
WORKMAN'S COMPENSATION: We do not accept any workers compensation or car injury patients or claims.
